As of October 1, 2023, HUD began transitioning from the old HQS inspection system to NSPIRE (National Standards for the Physical Inspection of Real Estate). By 2026, all PHAs must use NSPIRE. Here's what changed and how to pass.
🔄 What Is NSPIRE?
NSPIRE is HUD's modernized inspection protocol. It uses a numeric scoring system (0-100) instead of pass/fail for minor issues. Units must score 60+ points to pass, with automatic fails for certain health and safety hazards.

NSPIRE Scoring System Explained
Properties start with 100 points. Defects deduct points based on severity:
- Level 1 (Minor): -1 to -5 points (e.g., torn window screen)
- Level 2 (Moderate): -10 to -20 points (e.g., broken window, damaged flooring)
- Level 3 (Severe/Health & Safety): -25 to -50 points OR automatic fail (e.g., no smoke detectors, exposed wiring)

1. Smoke Detectors & Carbon Monoxide Alarms
NSPIRE Requirement: Smoke detector in EVERY bedroom, on every level (including basement), and outside sleeping areas. CO detector on each level with fuel-burning appliances.
Why it fails:
- Detector older than 10 years (check date on back)
- Missing from one bedroom
- Detector doesn't beep when tested
- Hardwired detectors without backup battery
Fix: Install interconnected smoke/CO alarms in all required locations. Test before inspection. Replace any units older than 10 years.
2. Lead-Based Paint Defects (Pre-1978 Properties)
NSPIRE Change: Inspectors now check MORE surfaces for peeling/chipping paint in pre-1978 homes.

- All window frames and sills (interior and exterior)

- Porches and railings
- Any surface accessible to children under 6

- All bedroom windows must open and lock
- Glass must be intact (no cracks > 1 inch)
- Screens required if windows open (vary by climate)

- Window painted shut (won't open)
- Missing or torn screens
- Broken locks
- Cracked windowpanes
Fix: Free stuck windows with a utility knife. Replace torn screens. Repair/replace broken locks.
7. Handrails & Safety Features
NSPIRE Requirement: Handrails required on stairs with 4+ steps.
Common fails:
- Loose/wobbly handrails
- Missing handrails on exterior steps
- Deck railings with gaps > 4 inches (child safety hazard)
Fix: Secure all handrails. Install missing rails. Repair loose deck balusters.
8. Kitchen & Bathroom Ventilation
NSPIRE Requirement: Kitchens and bathrooms must have working exhaust fans OR openable windows.
Common fails:
- Bathroom fan doesn't work
- Kitchen lacks ventilation (no fan, window painted shut)
- Range hood vents to attic instead of outside
Fix: Repair/replace broken exhaust fans. Ensure bathroom windows open if no fan is present.
9. Floors & Trip Hazards
What they check:
- Torn or loose carpet (trip hazard)
- Missing transition strips between rooms
- Cracked/broken tiles
- Holes in flooring
Fix: Tack down loose carpet. Replace missing transition strips. Repair cracked tiles or cover with vinyl.
10. Water Heater Safety
NSPIRE Requirement: Water heaters must have:
- Temperature Pressure Relief (TPR) valve
- Discharge pipe extending to 6-18 inches from floor
- Earthquake straps (in seismic zones)
- Clearance from combustibles
Fix: Hire a plumber to install TPR valve and discharge pipe if missing. This is a common DIY error.
How to Prepare for Your NSPIRE Inspection
2 Weeks Before:
- Schedule HVAC service and filter replacement
- Test all smoke and CO detectors
- Fix any known plumbing leaks
- Scrape and repaint peeling paint (pre-1978 properties)
1 Week Before:
- Replace missing outlet covers and light bulbs
- Repair or replace broken windows/screens
- Secure loose handrails
- Clean gutters and check exterior drainage
Day Before:

- Test all appliances (stove, oven, dishwasher)

What If You Fail?
Under NSPIRE, you have 24 hours to fix life-threatening items (exposed wiring, no heat in winter, sewage backup). For all other fails, you get 30 days.
Re-inspection process:
- Inspector provides deficiency report with point deductions
- You make repairs
- Schedule re-inspection (usually 7-14 days out)
- Inspector verifies fixes
- If unit now scores 60+, you pass
⚠️ Important: HAP payments are suspended if you don't complete repairs within the deadline. Payments resume (often retroactively) once the unit passes re-inspection.
